 We welcome self-referrals as well as referrals from family, carers, GPs, and other professionals. Choose the option that best suits you below

We accept referrals from GPs, case managers, and family and carers.

How to book:

If you would like to book an appointment for yourself, you can click the 'Book Now' button below or contact our Reception team at fusion@fusionhealthcare.com.au or phone 02 8014 4162.

What to expect:

Once you have booked your first appointment, our Reception team will be in contact with you to  confirm how you will be paying for your appointment (private, Medicare or other), and to finalise any other Intake details.


If you are using a GP Mental Health Treatment Plan (GPMHTP)  , or  a GP Chronic Condition Management Plan (GPCCMP), you will need to bring with you your GP referral, as well as your Medicare card to your first appointment.

How to book:

 We often receive referrals from family members, carers, and guardians who want to support someone they care about. 

If you would like to book an appointment for someone else, you can click the 'Refer now' button below to complete the referral form, or contact our Reception team at fusion@fusionhealthcare.com.au or phone 02 8014 4162.

What to expect:

When completing the referral form, you will need to provide the following information:

 

  • Client details (name, DOB, contact info)
  • Reason for referral (e.g., psychological support, physiotherapy, dietetics, occupational therapy)
  • Funding details (Medicare, private, other)
  • Referrer details (your relationship to the client)

For GPs, Health Professionals & Organisations

We accept referrals for GP Mental Health Treatment Plans, GP Chronic Condition Management Plans,  private health, and WorkCover.
